CASE MANAGEMENT
HRDC Homemaker Program

Spectrum Medical in partnership with the HRDC provides case management services under the Home and Community Based Services Program. The Case Management Team, consisting of nurses and social workers, serve the counties of Madison, Gallatin, Park and Meagher.

The Home and Community Based Services Program provides a variety of services in the home for persons who are Medicaid eligible, have a physician following their care, and meet nursing home level of care. Through this program the client has an active voice in the services to be provided and in the selection of the providers. The Home and Community Based Services Program is an alternative to nursing home care.

In concert with the State of Montana Department of Public Health and Human Services, the case management team coordinates and monitors the care provided to the person in their home to insure the services are appropriate, meet the needs of the client, are cost-effective and are of high quality.

The Homemaker Program assists low-income senior citizens (60 and older) with household chores, errands, shopping, and assistance for bathing and other personal care. Our goal is to provide our clients (who have limited resources) with an improved level of safety, personal hygiene, and a better quality of life while allowing them to remain independent in their own home for as long as possible.

Generally, the homemakers visit our clients one or two days a week for an hour or two. In that time they help with laundry, housecleaning, and shopping for food or other necessities. Our homemakers also assist our client with bathing in order to prevent falls and injuries. During times or crisis and/or temporary increased need, we may visit more often to offer help and support. Another important component of the program is connecting our clients with other available services within the community. Other agencies such as the Office of Public Assistance, Adult Protective Services, Meals on Wheels, the Gallatin Valley Food Bank, and the Mental Helath Center are often called on to assist if appropriate. The program is funded in part by the United Way and the Title III.
